Services

PRIMARY HOME CARE (PHC) is a Medicaid Program administered by the Texas Department of Aging and Disability Services.  PHC provides NON Technical Attendant services to Medicaid eligible clients who reside at home.
COMMUNITY ATTENDANT SERVICES (CAS)- Home attendant care services provided to persons with medical diagnosis that results in functional limitations.
FAMILY CARE SERVICES (FCS)-  Non-skilled attendant care services, such as home management or personal care services, available to persons with functional limitations to assist with activities of daily living
COMMUNITY BASED ALTERNATIVES (CBA) program provides home and community-based services for persons age 21 and older who would qualify for nursing facility care.  These services are provided as cost-effective alternatives to placement in NFs.  The goal of the CBA program is to provide individuals with meaningful choices regarding long term care services.  This goal will be accomplished primarily by facilitating the development and utilization of services which will allow individuals to avoid premature NF placement.

Heights Home Health also accepts VA, private pay, AAA, and private insurance clients.

Our services include but are not limited to: bathing, dressing, exercising, feeding, grooming, routine hair and skin care, toileting, transfer, ambulation, cleaning, laundry, meal preparation, escort to appointments, shopping, and assistance with self administration of medications.
